1. Advisory Role & Agency Relationship
Optavise and its affiliated entities operate as independent, state-licensed insurance agencies and brokers. We provide comparison rate quotes, advisory consultation, and enrollment facilitation for insurance products issued by third-party insurance underwriters. Insurance policies are legal contracts strictly between the underwriting carrier and the policyholder.
2. State-Regulated Premium Rates
All insurance premium rates displayed on this platform are filed with and regulated by state insurance departments. Rates are determined based on demographic criteria (age, gender, location, health classification, and tobacco status) and cannot be artificially marked up or discounted by agencies.
3. Government Program Disclosures (Medicare)
Not connected with or endorsed by the United States Government, the federal Medicare program, or the Centers for Medicare & Medicaid Services (CMS). Optavise does not offer every plan available in your area. Any information provided is limited to plans offered by underwriters with whom our counselors are appointed. Beneficiaries can contact Medicare.gov or 1-800-MEDICARE for full options.
4. Electronic Signature & Application Consent
By submitting an electronic application or quote request on our platform, you consent to receive electronic communications and confirm that all demographic and health disclosures provided are true and accurate to the best of your knowledge.
